Who can do ACP?
Anyone, regardless of age and without a life-limiting illness, can do ACP.
What are our fees?
We accept donations of any amount to help sustain our services and ensure everyone in the community can benefit from Advance Care Planning.
How to Apply?
We do not accept walk-ins. To schedule an appointment, please send us the following information via email swc@pcs.org.sg and our social worker will contact you
- Your name (including the names of any additional individuals if applying together)
- Your Contact number (for us to reach you)
- Your home address (for us to assign the nearest centre to meet)
In-person ACP sessions are available at the following locations: Bukit Merah, Tampines West, Upper Serangoon, Queenstown
