Job Description
Oversee daily operations and administrative matters, work with volunteers, grassroots, schools, public institutions and other social services agencies on activities and programmes for the Seniors within the service boundary which includes:
- Outreach and engagement of seniors within service boundary
- Active ageing programme
- Befriending and Buddying
- Care and support services
- To liaise with external agencies, organisations, donors, volunteers etc. to tap on external resources and / or publicise programmes. You will need to provide information and referral services to clients.
Roles and Responsibilities
1. Programme Planning and Implementation
- Coordinate support services and organize activities for the seniors in the identified service boundaries
- Develop and implement outreach plan to seniors.
- Develop and implement annual calendar plan of social and health activities for seniors.
- Ensure that the programmes and activities meet the performance indicators and operating guidelines issued by Agency for Integrated Care, administration of the CSQ, half-yearly progress report on the programmes and services, outcome management, etc.
- Conduct needs assessment survey if necessary.
- Initiate a case record file for every elderly household in the identified block and capture the needs of the seniors.
- Establish and motivate seniors to be active, healthy, interdependent and purposeful.
- Ensure that the activities and programmes of the centre promote a community climate of multi-racial tolerance, harmony and co-operation among residents.
- Network with other agencies to establish additional avenues of help for the welfare of the seniors
2. Alert Alarm System
- Oversee and manage the Alert Alarm System (AAS) during the centre’s operating hours, rendering assistance when necessary.
- Establish a system to manage the AAS outside operating hours and ensures that help continues to be available when needed, including a system of recalling staff outside operating hours
3. Volunteer Development
- Establish networks with local community-based services/ resources, such as grassroots, social services, corporate organisations and schools, to engage volunteers to help run or assist in the programmes, start income generating activities, conduct outreach efforts etc., to match the needs of seniors.
- Recruit, interview and screen potential volunteers.
- Provide and co-ordinate orientation of volunteers to their assigned role and the mission and objectives of the centre.
- Place, evaluate volunteers to suitable positions and areas of need, such as administration of CSQ etc.
- Supervise and support volunteers.
- Establish a system of recognition for the volunteers.
- Review and evaluate the volunteer programme to ensure relevance and to meet future needs.
- Prepare timely report on AAC programmes and activities to the management and funders of the programme.
- Serve as the key communication link between the centre and its volunteers.
4. Finance and Accounts
- Manage the monthly petty cash utilization of the centre.
- Implement checks and controls in the accounting system to ensure proper accounting records and also to safeguard against misappropriation of the centre’s assets.
5. Training and Development of Personnel
- Lead, motivate and train junior personnel.
6. Administrative Duties
- Provide secretariat support to the Division of Senior Services.
- Undertake any other duties as required.
Qualifications
- Diploma in Social Service or GCE ‘A’ level with at least 2 years of related working experience
- Possess experience in running operations or Active Ageing Centres
- Possess passion and empathy in helping the elderly

Reports to the DS-LS Programme Manager
Role
1. Case Management
- Facilitate parents’ understanding of programme and needs of child from screening and recommendation(s) from relevant professionals and sources in the centres
- Collate and consolidate all information about the child and family from relevant professionals and sources in the school
- Facilitate social work-parents meeting with the DS-LS team to share assessment outcomes of child and recommendations/next steps required
- Facilitate all documentation needed for enrolment into the programme
- Identify and track areas for follow up (e.g. referral to internal/external resources)
2. Documentation and Administrative Tasks
- Prepare for and participate in Case Filter Meetings, take notes and maintain good record keeping
- Complete documentation (e.g. case notes) accurately and promptly for other professionals who are working with child
- Maintain accurate records, prepare reports for governing agencies, collate information and submit government means-test applications
3. Collaborative Partnerships/Community Networking
- Utilise appropriate and available community resources to support child and/or family
- Network and work collaboratively with various community agencies (e.g Family Service Centres, Social Services Offices or other agencies) for collaborative problem-solving
Requirements
Technical Competencies:
- At least a Diploma in Social Work (Social Work Associate)
- 1 to 3 years of experience working with families and children preferred (fresh graduates may apply)

About The Role
This role is responsible for the day-to-day accounting to support the community-based services and programmes of Presbyterian Community Services (“PCS”). The main responsibilities include:
- Producing timely and accurate financial statements and management reports
- Maintain accounting records and ensure finance processes comply with PCS policies and statutory requirements. This includes ensuring supporting schedules are up to date and accurate and issues arising are resolved promptly
- Ensure proper monitoring and tracking of expenditure and KPIs by individual programmes to ensure timely submission and compliance with government agencies/grants/programmes/legal/statutory requirements.
- Ensure donations are processed timely and accurately, including submission of tax deductible donations records to IRAS, reconciliation of donation management system with accounting system
- Maintain full set of accounts for Partners, monitor and keep PCS management abreast of Partners’ financial performance and cash-flow
- Assist in the preparation of annual budget including monthly tracking of actual expenses vs budget and implementation of other budgetary controls

Technical Competencies

He/She will participate in case consultations and provide relevant support services to benefit clients and their families. He/She will deliver the practice in a professional and ethical manner with due consideration to the relevant legislation and procedures as well as to the ministry’s requirements. Some of the key responsibilities include:
- Provide financial assistance services to needy clients in accordance with the organisation’s financial assistance scheme policies
- Conduct family and social assessment of client’s needs when necessary
- Provide crisis intervention support in line with the organisation’s policies, including recommendations for financial assistance
- Plan, implement, supervise and evaluate the intervention services to meet the needs of the clients
- Undertake casework, community work & counselling
- Work with external agencies e.g. ministries & hospitals for the effective programme development and management

Job Description
The Occupational Therapist works with fellow allied professionals, teaching staff, centre staff and parents/caregivers to provide occupational therapy intervention (assessment, intervention, consultation and evaluation) to clients. He/She will participate in case consultations and provide relevant support services to benefit clients and their families. The standard of professional practice has to be ethical and delivered with due consideration to the relevant legislation and procedures, as well as ministries requirement. Some of the key responsibilities include:
- Assess and identify individual Occupational Therapy (OT) needs of clients
- Formulate/develop and implement appropriate individualized OT intervention plans or programmes
- Evaluate, monitor and document client’s OT needs and progress at regular intervals
- Work with clients on a one-to-one or group setting based on their levels of functioning
- Provide clear, relevant and practical home/centre strategies to caregivers and centre staff
- Maintain proper usage of OT equipment/resources and recommend replacement or purchase of equipment/resources as needed

Job Description
The Enrolled Nurse performs all nursing procedures which include administration of medications, wound dressing and meal feeding. He/she assists the Senior/Registered Nurse during treatment of clients and supervise clients in medication packing/insulin injection after assessment by Senior/Registered Nurse. Some of the key responsibilities include:
- Assist Senior Nurse in developing a care plan (goals & action plans), carry out the care plan and regularly evaluate the plan
- Carry out overall hygiene care and Activities of Daily Living (ADL) support for clients
- Accompany clients in participation of their daily group activities
- Assist in maintaining clients’ records on their condition and progress
- Provide client’s family members with feedback on the client’s health conditions and help them gain understanding on the holistic care provided (e.g. educate them on healthy living)
